We understand it can be confusing to work out who you need to book an appointment with for certain problems. We have produced a leaflet for patients to give some idea of who to book in with to make things a bit easier. Sharing Your Medical Record

Summary Care Record (SCR)

The Summary Care Record (SCR) is designed to improve the level of care patients can receive, especially in an emergency situation. The SCR will enable NHS clinicians to access basic information regarding the patient's current medication and any allergies.   For example, if a patient was to be taken ill whilst on holiday in the UK, a Doctor would be able to view details of their current medication and any allergies, ensuring that suitable and appropriate treatment is administered. There is now also the option of having an enhanced summary care record. This will include the information above, as well as information on your illnesses and health problems, operations and vaccinations you have had in the past, how you would like to be treated (such as where you would prefer to receive care), what support you might need and who should be contacted for more information about you.

We cannot enable your enhanced SCR without your given consent, either verbally or by completing the 'consent for additional SCR' form below.

Ask a receptionist at the surgery or visit;

All patients have the choice to ‘opt out’ of this system, ensuring that this information is not available anywhere else. Obviously, this would mean that healthcare professionals cannot share information about patient's medications and allergies should an emergency situation arise.

If you wish to opt out of the Basic Summary Card Record scheme, then you will need to complete a form and bring it along to the surgery.

Your Summary Care Record

If you already use SystmOnline to book appointments and order repeat medications you can now also see a summary of your medical record. This shows any allergies or sensitivities; any acute prescriptions you have had over the last 12 months and your current repeat prescriptions.

If you wish to access these online services call at Reception next time you are in the Surgery. You will need to show two forms of ID - one with a picture on such as a passport or driving licence, and another with your address on, such as a utility bill. You will then be given a letter with details of how to log on to SystmOnline.

Enhanced Data Sharing Model (eDSM) – Sharing Out 

Sharing out means that any information within your medical record (all consultations, results, medication etc.) will be available for other NHS organisations to see if you allow them to. This will make all information sharable.

You can always change your mind, but if you have chosen to share information with other NHS organisations and then decide against it, we cannot retrieve any information already shared.

If I choose to make my medical record shareable, I understand this means that ALL consultations recorded in my record will be available for other NHS services to view.

Enhanced Data Sharing Model (eDSM) – Sharing In

Sharing in means that Posterngate Surgery will be able to see all information added by other healthcare organisations. This will only happen if you have consented to share information out at the other organisation.

We have a set list of organisations who, once you give consent to share in/out, will be able to view your record. If an organisation is not in the list that we have created and you want them to view your record, you may be required to give them a verification code. When the organisation requests this access, you will be sent a code either via SMS, email or on your SystmOnline account. For more information in regards to this, please ask at Reception.
