Appointments at Posterngate Surgery

We understand it can be confusing to work out who you need to book an appointment with for certain problems. We have produced a leaflet for patients to give some idea of who to book in with to make things a bit easier. Registering with the practice

PreRegistrationIf you wish to register with the practice you will need to come into the surgery to collect the paperwork you are required to complete.

In addition, we also need you to provide proof of identification (preferably photo ID) such as a passport or drivers licence.  You will also need to provide proof of your address, such as a recent utility bill.

When returning your new patient forms you will be required to take a seat in the waiting room whilst the Reception Team register your details. As this is a new registration system for the practice, please allow sufficient time for us to accurately complete this process and address any queries which may arise with you.


Summary Care Record - your emergency care summary

The NHS in England has introduced the Summary Care Record, which is used in emergency care situations. 

The record will contain information about any medicines you are taking, allergies you suffer from, and any bad reactions to medicines you have had, to ensure those caring for you have enough information to treat you safely.  This information will be available to authorised healthcare staff providing your care anywhere in England.

Enhanced Data Sharing Model (eDSM)

The eDSM is a programme whereby patient records can be shared across other parts of the NHS.

It is different from the ‘summary care records’ which aims to share important data, such as your medication history, sensitivities and allergies.

This new change will mean that subject to your consent, all your GP records, including every recorded consultation, can be shared.  If you agree, your GP records will be shared out into a pool and other units, again with your permission, can view all your clinical records.
